Participate in Anna Banana's research

In Maastricht Anna Banana handed me a research form which she asked me to fill in. She gave me an extra form to digitize and to ask people to print it and fill it out to to return to her. The more responses, the better the research will be. On the first page you find the address to send the sheet back to. Hope you will join in.
