pLEAse Help--think here?

I have had this for the longest time. Problem is, I sent it to the person it was thought to have originated with (i can't remember now who it was) but it was returned to me undeliverable. This was worked in by Alicia Starr, Samuel Montalvetti, me, and Antonio Moreno. Does anyone know where i should send it?
