DKult Health Insurance Issues

DK herself sent this great package: It seems that there are more formalities for the common DKult member than I realized before! Now I have to fill out this:

Has anyone experience with this kind of commitment to DKult? I would be grateful to know more about it before selling my soul (or does it already belong to Diane? I'm not sure)
